This intake form helps me get a sense of where you're at before your session — in your body, your mind, and your life in general. Be as honest as you want and don't worry about saying the "right" thing. Some things may seem irrelevant, but any information or insight you provide will be helpful in deeply understanding You as an individual, unique and whole person.
